Sandy and storm surge pose 'worst case scenario'

KENSINGTON, Md. (AP) ? The projected storm surge from Hurricane Sandy is a "worst case scenario" with devastating waves and tides predicted for the highly populated New York City metro area, government forecasters said Sunday.

The more they look, the more the experts worry about the water ? which usually kills and does more damage than winds in hurricanes.

In this case, seas will be amped up by giant waves and full-moon-powered high tides. That will combine with drenching rains, triggering inland flooding as the hurricane merges with a winter storm system that will worsen it and hold it in place for days.

Louis Uccellini, environmental prediction chief for the National Oceanic and Atmospheric Administration, told The Associated Press that given Sandy's due east-to-west track into New Jersey, that puts the worst of the storm surge just north in New York City, Long Island and Northern New Jersey. "Yes, this is the worst case scenario," he said.

In a measurement of pure kinetic energy, NOAA's hurricane research division on Sunday ranked the surge and wave "destruction potential" for Sandy ? just the hurricane, not the hybrid storm it will eventually become ? at 5.8 on a 0 to 6 scale. The damage expected from winds will be far less, experts said. Weather Underground meteorologist Jeff Masters says that storm surge destruction potential number is a record and it's due to the storm's massive size.

"You have a lot of wind acting over a long distance of water for hundreds of miles" and that piles the storm surge up when it finally comes ashore, Masters said. Even though it doesn't pack much power in maximum wind speed, the tremendous size of Sandy ? more than 1,000 miles across with tropical storm force winds ? adds to the pummeling power when it comes ashore, he said.

The storm surge energy numbers are bigger than the deadly 2005 Hurricane Katrina, but that can be misleading. Katrina's destruction was concentrated in a small area, making it much worse, Masters said. Sandy's storm surge energy is spread over a wider area. Also, Katrina hit a city that is below sea level and had problems with levees.

National Hurricane Center Director Rick Knabb said Hurricane Sandy's size means some coastal parts of New York and New Jersey may see water rise from 6 to 11 feet above ground from surge and waves. New York will close its subways starting Sunday evening. The rest of the coast north of Virginia can expect 4 to 8 feet of surge.

Knabb said millions of people may be harmed by inland flooding.

A NOAA map of inland and coastal flood watches covers practically the entire Northeast: All of Maryland, New Jersey, Delaware, and Connecticut; most of Pennsylvania, New York, Massachusetts and Vermont, and parts of northeastern Ohio, eastern Virginia, North Carolina, and western New Hampshire.

Along the mid-Atlantic coast, storm surge is already starting to build, Uccellini said. NOAA's Coastal Services Center chief Margaret Davidson said to expect "bodacious impacts" in the area.

The surge ? in which water steadily increases from the ocean? will be worst in the areas north of where Sandy comes ashore.

New York will have the most intense storm surge if Sandy comes ashore anywhere in New Jersey, Uccellini said. Only if it arrives farther south, such as in Delaware, will New York see a slightly, only slightly, smaller storm surge.

In general, areas to the south and west of landfall will get the heaviest of rains. Some areas of Delaware and the Maryland and Virginia peninsula will see a foot of rain over the several days the storm parks in the East, Uccellini said.

The good news about inland flooding is that the rivers and ground aren't as saturated as they were last year when Hurricane Irene struck, causing nearly $16 billion in damage, much of it from inland flooding in places like Vermont, Uccellini and Masters said.

Sandoval homers off Verlander in Game 1

So far, Barry Zito has the Tigers off stride a bit. He's been able to get inside on the right-handers and he's using all his pitches. Getting groundballs and keeping the ball off the barrel of the bat.

Justin Verlander threw a nasty curve to get Buster Posey on a called third strike to end the first inning. Then he got Hunter Pence to wave at a slider for strike three to start the second. The reigning AL MVP and Cy Young Award winner appears to be settling in, finding his breaking ball.

This is the fourth time that Cy Young winners have squared off in Game 1 of the World Series. Zito won his AL Cy Young Award with Oakland 10 years ago. The guys on FOX said that's the longest gap for a pitcher between winning the Cy Young and making his first World Series start.

It's the All-Star game revisited! Pablo Sandoval hit a bases-loaded triple off Verlander in the first inning of the All-Star game in Kansas City back in July.

Now, Sandoval has a World Series homer off the Tigers' ace.

Verlander struggled through a five-run first inning in the All-Star game, throwing 35 pitches. The National League won 8-0, giving the Giants home-field advantage in this World Series.

Turns out, Verlander has a nemesis.

Barry Zito's average fastball velocity ranks among the slowest in the majors, and he's not throwing any harder to start the World Series. The veteran lefty was hitting 84-85 mph on the radar gun, but he hit his spots when he needed to in the first inning.

Omar Infante singled with one out and Zito walked Triple Crown winner Miguel Cabrera on a close 3-2 pitch. With two runners on, Prince Fielder popped out on a tough pitch down and away, and ALCS MVP Delmon Young grounded into a force play on a down-and-in breaking ball.

Clutch pitching by Zito against the dangerous middle of the Detroit lineup. You don't want to fall behind early against Verlander.

Verlander's first three pitches were 94 mph fastballs. He looks as though he's trying to settle himself down a bit and calm his emotions as he fiddles with his glove and the rubber.

Here we go! All set for baseball's 108th World Series.

Game 1 is tonight in San Francisco, with the NL champion Giants hosting Justin Verlander and the AL champion Detroit Tigers. First pitch about 15 minutes away.

It's a late afternoon start on the West Coast, so it will be interesting to see if early shadows on the field are a factor. If so, they could make it awfully tough for the Giants to catch up with Verlander's heat.

Gerry Davis is the plate umpire. Generally, the book on him is that he has a fairly tight strike zone. Tougher on pitchers.

That might make things more difficult for Zito. He throws a lot of soft stuff and may need to get a few calls around the corners.

With his team facing a left-hander, Tigers manager Jim Leyland moved second baseman Omar Infante up to No. 2 in the lineup and dropped catcher Alex Avila to eighth.

Flipboard on iOS has had audio for some time, giving users the chance to go all high-brow as they listen to NPR while browsing the news on their iPad. Adrian Peterson probably wishes the Vikings were in the NFC West.

The last two times he has faced the Arizona Cardinals, Peterson has been named the offensive player of the week in his conference.? For his 153 rushing yards on 23 carries and one touchdown on Sunday against Arizona, Peterson secured his sixth career player of the week award.

Last year against the Cardinals, Peterson scored three first-quarter touchdowns and gained 122 rushing yards in winning the weekly award.

Packers fans may disagree with the selection, given that Green Bay quarterback Aaron Rodgers completed 30 of 37 passes of (81.2 percent) for 342 yards and three touchdowns, good for a 132.2 passer rating.

Through seven games, Peterson has 652 rushing yards, three touchdowns, and zero (yes, zero) lost fumbles.? His performance against the Cardinals gives Peterson 29 career 100-yard rushing games, tying him with Robert Smith for the most in franchise history.

Need CPR? Whether you get it may depend on the neighborhood

If your heart stops and you fall to the ground, your chances of survival may depend on which neighborhood you're in when you collapse.

Patients suffering cardiac arrest in poorer, predominantly black neighborhoods were half as likely to receive CPR from a bystander as those in richer, predominantly white neighborhoods, according to research published in Thursday's edition of the New England Journal of Medicine.

Even cardiac arrest victims in well-to-do black neighborhoods were 23% less likely to receive bystander assistance. And overall, blacks and Latinos were less likely to receive aid, regardless of where they were.

"We've started to look into how neighborhoods affect people's health for obesity and food deserts and cancer, but this is the first time we've really thought about it for cardiac arrest," said Dr. Comilla Sasson, an emergency medicine physician at the University of Colorado School of Medicine in Aurora, who led the study.

Sudden cardiac arrest outside a hospital setting is the leading cause of death in the United States, killing some 300,000 people annually. Sudden cardiac arrest is caused by an electrical glitch that prompts an abnormal heart rhythm and is different from a heart attack, which happens when blood flow is blocked and a region of the heart is starved of oxygen. About 92% of people who suffer sudden cardiac arrest die.

This is where CPR comes in. Those first few minutes after a person's heart malfunctions, but before an ambulance arrives and emergency medical responders take over, can mean all the difference for survival.

But survival rates vary widely from city to city. In places like Seattle, the rate is 16%. In Detroit, it's 0.2%.

It's unclear exactly why these differences exist. Seattle is thought to have a widespread culture of CPR training, Sasson said, and it's somewhat whiter and more well-to-do than long-suffering Detroit. Studies in Canada and Seattle have indicated that economics are key, with victims faring better in richer areas than in poorer ones. But studies conducted in Chicago in the 1980s found that race was a major factor.

The researchers behind the new analysis suspected that both issues were in play. They culled data from 29 cities and counties covering some 22 million people, using data from the Cardiac Arrest Registry to Enhance Survival, which is maintained by the Centers for Disease Control and Prevention.

Sasson and her colleagues examined 14,225 cases of sudden cardiac arrest from October 2005 through December 2009 and used census-tract data to sort the victims according to where they had their attack. The categories included neighborhoods that were "rich" (with a median income of $40,000 or more) or "poor" (below $40,000), as well as those that were more than 80% black, more than 80% white or integrated.

The researchers found that, overall, 28.6% of people received CPR from a bystander. But their particular odds of receiving help depended greatly on where they were.

Compared with victims in high-income, predominantly white neighborhoods, those in low-income, predominantly black neighborhoods were 51% less likely to receive CPR, and those in poor white neighborhoods were 35% less likely to receive CPR, the team calculated.

Even those who collapsed in rich black neighborhoods were 23% less likely than their peers in rich white neighborhoods to receive CPR from a bystander. There was no significant difference in CPR rates for victims in rich integrated neighborhoods compared with those in rich white neighborhoods.

Add this to the fact that some of the communities with a very high risk of sudden cardiac arrest are also among those least likely to provide bystander CPR and you've got a prescription for some very concerned cardiologists.

But the study also found that, overall, blacks and Latinos were about 30% less likely than whites to receive CPR from a bystander no matter where they were when they fell ill.

Why? "That's sort of a sociologist's question," said Dr. Robert Berg, chief of critical care medicine at the Children's Hospital of Philadelphia. "As a physician that's involved in cardiac arrest and resuscitation, that's disappointing to me."

For their part, the medical professionals focus on the aspects they can address.

Part of the reason for the discrepancy in CPR rates has to do with cost, said Dr. Gordon Ewy, director of the University of Arizona Sarver Heart Center in Tucson.

"If you are struggling to make ends meet, you don't have time to spend $45 and several hours 'getting certified,'" said Ewy, who was not involved in the study.

And for many, Sasson said, the perceived crime risk in low income, minority neighborhoods keeps them from going out in the street to help someone who has collapsed.

That's in part why emergency medical physicians and cardiologists are promoting a new, simpler type of CPR. It requires no mouth-to-mouth resuscitation, only that a bystander perform at least 100 chest compressions per minute, to the beat of ? yes ? the Bee Gee's disco-era hit "Stayin' Alive."

The hope is that people will be more willing to perform this kind of CPR, and more confident too. It requires no certification; viewers can learn the technique by watching videos on YouTube.

"It's now up to us in the medical community and the public health community to take that data and to shift our way of looking at how we deliver CPR education," Sasson said.

Egg Freezing Enters Clinical Mainstream

The American Society for Reproductive Medicine has issued guidelines, which effectively greenlight the procedure for fertility clinics

Egg freezing is no longer an experimental procedure, according to the American Society for Reproductive Medicine (ASRM), which on 22 October issued new guidelines on the controversial practice. The change in policy is expected to accelerate the growth of clinics that offer egg freezing to women who face fertility-damaging treatment for cancer or other conditions, and to women wishing to delay having a baby ? although the society stopped short of endorsing the procedure for that purpose.

More than 900 babies have been born using the technique, which the ASRM called ?experimental? in 2008. With that designation, the society approved of the use of egg freezing only in clinical trials overseen by an institution review board (IRB). Despite the ASRM policy, clinics have increasingly been offering the technique outside of this framework as a clinical service for a fee. Now the society is effectively giving such clinics a green light, a development that is likely to encourage consumer groups advocating for insurance providers to cover the procedure.

?This will open up the procedure for many, many people,? says Samantha Pfeifer, an associate professor of obstetrics and gynecology at the University of Pennsylvania, in Philadelphia, who headed the committee charged with making the recommendation. She adds, ?There are enough preliminary data to say that this technique is safe.?

The committee removed the ?experimental? label after analyzing more than 100 studies assessing the health of eggs after the procedure and the outcome of births. The committee concluded that there have been no reported increases in chromosome abnormalities or birth defects among children born using oocyte cryopreservation, the technical term for the procedure.

But the committee also noted that the data so far are incomplete ? only four randomized clinical trials have been conducted using the technique. And while those studies suggest that the technique results in successful pregnancy in young women at the same rate as assisted reproduction with ?fresh? oocytes, observational studies suggest the success rate is lower. Because the technique of freezing the large and fragile cells is relatively new, nothing is known about long-term effects on offspring that result from the method. Some studies suggest that babies born using assisted reproductive technologies are at an increased risk for certain rare disorders ? the neurological condition Angelman syndrome, for example ? that result from changes in the chemical modifications to DNA known as ?epigenetic marks?.

The safety and success rates of the procedure have been the focus of heated debates among reproductive biologists. At the ASRM annual meeting in San Diego this week, for instance, Pfeifer will be facing off against Nicole Noyes, head of a clinic affiliated with New York University that offers the procedure. The topic will be whether younger women should consider freezing their eggs for the future, says Pfeifer.

Despite the need for more research, the new guidelines will help clinicians counsel patients about the technique?s safety and potential to result in a pregnancy, says Theresa Woodruff, chief of the Division of Fertility Preservation at the Feinberg School of Medicine at Northwestern University in Chicago, ?It?s a good, measured step,? she says. The society announced its new policy on 19 October and published its new report yesterday in the journal Fertility and Sterility.

There were not enough data for the committee to recommend the procedure for women nearing the end of their reproductive years who wish to preserve their eggs for future use, says Pfeifer. And the data so far suggest that the success rates of the procedure decline with maternal age. Nonetheless, the use of egg freezing for delaying reproduction is a potentially huge growth area ? many clinics are already offering these services.

SYDNEY/SEOUL/SINGAPORE, October 23 (Fitch) The government of Canada's decision to block the takeover of Progress Energy Resources by Malaysia's PETRONAS ('A'/Negative) illustrates the rising global trend of resource nationalism.

Governments are under pressure to maximise control of their natural resources as a buffer against the rise in commodity prices over the past 10 years, stalling global economic growth and the ultra-easy monetary policies adopted by major central banks including the US, Europe and Japan.

This decision increases the likelihood that the Canadian government will also reject China's CNOOC Ltd's (CNL, 'A+'/Stable) bid to acquire 100% of Nexen, a Canadian upstream oil and gas producer, for an equity value of CAD15bn.

The Canadian government's decision to block PETRONAS's CAD5.5bn takeover was notwithstanding the fact that PETRONAS had offered a 77% premium to Progress's pre-bid share price - which had been approved by the shareholders.

Canada's industry minister commented that the government "was not satisfied that the proposed investment is likely to be of net benefit to Canada."

Details behind the government's decision are not available, but PETRONAS has 30 days to modify its proposal before the government's decision is finalized. Progress has indicated its willingness to consider the issues behind the rejection, but both Progress and PETRONAS claimed in June 2012 that the deal would provide a net benefit to Canada. Fitch Ratings believes possible reasons for the rejection include the government's desire to ensure that sufficient control and profits remain in Canada, as well as guaranteeing local jobs.

This is in line with the current global wave of "soft" resource nationalism, in which governments are opting to gain a greater share of resource company profits through changing regulation, taxation and contractual terms.

The deal with Progress would have provided PETRONAS with an opportunity to enlarge its reserves, diversify its product range, and potentially commence LNG exports from North America to Asia. Tapping higher-priced and strong LNG demand from Asian utilities would be a strategic alternative for Progress away from its traditional outlet of the North American pipeline gas market.

Progress Energy is an upstream gas producer owning gasfields in the Montney shale areas of British Columbia and Alberta. The company has been unable to expand due to the relatively small size of its operations and financial resources required for development.

Fitch believes that PETRONAS's ownership would provide the required infrastructure investment to commence exports of LNG to Asia. In September, Fitch affirmed PETRONAS's Long-Term Foreign-Currency Issuer Default Rating (IDR) at 'A', which remains one notch above that of Malaysia at 'A-'.

However, the agency changed the Outlook on PETRONAS to Negative, as growing government influence over the company's financials and strategy weakens the case for this state-owned entity to be rated above that of the Malaysian sovereign.

Former premier Berlusconi says he won't run

MILAN (AP) ? Former Italian Premier Silvio Berlusconi announced Wednesday that he won't run for a fourth term in spring elections and said his center-right movement should hold a primary to choose its candidate.

"I won't run for premier," Berlusconi said in a statement posted on his movement's website, "but I will remain next to younger players who need to play and score goals."

Berlusconi, who stepped down in November after failing to come up with effective economic reforms to shield Italy from the debt crisis, has until now been coy about his intentions. But the soccer metaphor also indicates that the 76-year-old media mogul, and owner of the soccer club AC Milan, isn't ready to give up politics entirely.

"I still have good muscles, a bit of a head, and my role is to give advice, offer memory, stories and judgments without being intrusive," Berlusconi said.

Berlusconi recommended that his People of Freedom movement hold primaries on Dec. 16 to decide a candidate for premier. Berlusconi last year had named Angelino Alfano as his successor to head the political movement, but Alfano recently has been relegated to supporter role as Berlusconi publicly mulled his options.

Wednesday's announcement was met by a flood of political reaction, including those who doubt this will be the last word.

"For now, that is just one of the many Berlusconi announcements to which he has accustomed Italians," said Italo Bocchio, a former Berlusconi ally whose party defected from the movement in a bitter split. "We'll see in a short time if it is something concrete, or only a propagandistic discovery."

Maurizio Sacconi, a former minister in Berlusconi's Cabinet, called the move "courageous and generous," one that will allow the "moderates" on the right to unite behind a leader. Sacconi said he would throw his support to Alfano.

Matteo Renzi, the Florence mayor who plans to challenge Democratic Party leader Pierluigi Bersani in center-left primaries, welcomed Berlusconi's push for a primary elections to decide his successor on the center-right. "I am very happy for Italian democracy," Renzi wrote on his Facebook page. "It is a significant step forward."

Berlusconi came to power in 1994, and he dominated Italy's political scene until he was forced to resign last November with the country in financial shambles and a personal legacy tarnished by sex and corruption scandals. He currently is on trial in Milan on charges of having paid an under-age Moroccan teen for sex and trying to cover it up.

Berlusconi was replaced by Premier Mario Monti, who heads a government of technocrats charged with reforming Italy's economy to stave of the continent's sovereign debt crisis. The government's mandate expires in the spring. Monti has ruled out running for the post, but has signaled a willingness to stay on for another term in case no political party commands a majority and politicians ask him to stay on.

The Coca-Cola Foundation Awards $2.48 Million to Support Local Initiatives, One-Third to Active Living Programs

    2 off-duty Fla. officers fatally shoot naked woman

    TAMPA, Fla. (AP) ? Two off-duty Florida law-enforcement officers fatally shot an armed, naked woman who confronted them at a social gathering Saturday, authorities said.

    The shooting occurred at about 1:15 p.m. Saturday in Hernando County, north of Tampa. The county sheriff's office said in a news release that the men were approached by an "armed, naked and irrational female." The news release does not identify the weapon, but it says "one or both of the law enforcement officers fired their weapons, striking the female." She died at the scene.

    Both officers have been placed on administrative leave as the Florida Department of Law Enforcement investigates the shooting in the Spring Hill community.

    Authorities have identified the officers as Hernando County sheriff's Detective Rocky Howard and Tampa police officer William Mechler, who have served six and four years, respectively. Tampa police say Mechler was "off duty and acting in the capacity of a private citizen."

    Lt. Cinda Moore, a spokeswoman for the Hernando sheriff's office, said in an email late Saturday that she could not immediately identify the woman or disclose the weapon she was carrying. She said she also could not comment on what the woman specifically said or did or whether she was told to drop the weapon, explaining that the FDLE is leading the investigation.

    The Hernando County sheriff's office does permit the carrying of service weapons when off-duty, she said.

    The Tampa-area shooting comes two weeks after a naked, unarmed University of South Alabama freshman was fatally shot by a campus police officer after going to the police station there and pounding on the glass. The shooting has led to protests by students.

    Police said 18-year-old Gil Collar had taken LSD and acted aggressively when an officer came outside carrying a gun. The officer repeatedly backed away from the student before firing. An attorney for the student's family says Collar never touched the officer and the fatal shooting never should have happened. Authorities have said a grand jury will review that shooting to determine whether charges should be filed.

    Feds: Steelers ex-doc prescribed illegal steroids

    PITTSBURGH (AP) ? A former Pittsburgh Steelers doctor who left the team after investigators questioned his bulk purchase of anabolic steroids has been indicted for illegally prescribing the muscle-building aids and other drugs.

    Federal authorities don't list the team or its players as having gotten the substances from Dr. Richard Rydze, who was charged in a 185-count indictment with conspiracy to illegally distribute steroids, human growth hormone and painkillers including oxycodone.

    The 62-year-old physician also is charged with health care fraud. Authorities said he falsely diagnosed more than 90 patients with pituitary dwarfism so they could receive human growth hormones and drugs meant to counteract the side-effects of steroid use. The FBI contends the patients were normal-sized adults who didn't need such substances.

    Rydze and his attorney declined to comment after his initial appearance before a federal magistrate in Pittsburgh on Friday afternoon. The doctor will remain jailed until Monday, when U.S. Magistrate Judge Robert Mitchell must decide whether Rydze should stay behind bars until trial.

    Assistant U.S. Attorney Carol Skutnik told Mitchell she will ask for continued detention because Rydze has continued to prescribe drugs "10 to 15 times" in recent weeks despite losing his federal license to do so in July.

    Public defender Michael Novara told Mitchell he believes Rydze should be freed because the doctor is a "lifelong, law-abiding, well respected member of the community."

    Rydze has been under a legal microscope before.

    Prosecutors in Albany, N.Y., said he was on the customer list of an Orlando, Fla., pharmacy that was raided in February 2007 as part of an interstate steroids ring. Rydze was questioned then about buying $150,000 worth of testosterone and human growth hormone on his credit card in 2006, but was not charged in that investigation.

    After the Florida raids, Steelers president Dan Rooney issued a statement saying: "There is no evidence that Dr. Rydze prescribed or provided any hormone treatments to any of our players. Dr. Rydze has assured me that this has never happened and will never happen."

    Rooney also said then the team would "continue to monitor this situation to make sure that we can continue to feel confident in our medical staff in this area." The team dropped Rydze from its roster of doctors without explanation in June 2007.

    Steelers spokesman Burt Lauten said Friday that the team wouldn't comment on the charges against Rydze but stands by Rooney's statements five years ago.

    Rydze's patient base was so broad in Pittsburgh that federal authorities took the unusual step of having FBI agents from Ohio investigate, said Michael Tobin, a spokesman for the U.S. attorney in Cleveland. Prosecutors from Cleveland will also try the case, though that will occur before a federal judge in Pittsburgh, Tobin said.

    "He was an everybody-in-town-knows-him type of guy," Tobin said, explaining the decision to use outside investigators.

    Tobin declined to comment on whether Rydze's ties to the Steelers played a role in that decision.

    The indictment doesn't list the names of any patients, and prosecutors wouldn't say if Rydze illegally supplied players with steroids or other drugs.

    "We aren't saying anything one way or another, and the indictment doesn't say one way or the other, either," Tobin said.

    Assistant U.S. Attorney Steven Dettelbach issued a statement saying Rydze used "his prescription pad like a personal ATM, doling out steroids, painkillers and other medicine for his own gain." Investigators found Rydze personally received $301,000 in commissions from one pharmacy he worked with and his practice received $146,000.

    Rydze is charged with conspiring with James Hatzimbes, 42, to distribute steroids and human growth hormone from September 2007 through March 2011. The doctor also is charged with illegally prescribing painkillers to another man, William Zipf, 56, and his relatives during a slightly longer time frame, March 2007 through January of this year.

    Hatzimbes ran a business called HSE Salon and Wellness Center in Pittsburgh and worked with Rydze to schedule "steroid clinics" every other week at which people were falsely diagnosed with conditions that required steroids and growth hormones.

    Online records don't list attorneys for the co-defendants, and the number to Hatzimbes' spa was disconnected Friday.

    Hatzimbes is also one of the people Rydze allegedly diagnosed with dwarfism. The FBI said Hatzimbes actually is 6 feet tall.
